General
information for passport applicants |

1.
The validity of a passport cannot be extended beyond the date of its
final validity. An application for reissuing a passport has to be submitted
in the prescribed form along with the old passport and a copy thereof. |

2.
A separate passport must be obtained by every individual. A child's
name can no longer be endorsed on a parent's passport. |
|
3.
The applicant should submit the application in person for identification. |
|
4.
Two recent photographs (4x4cm) showing frontal view of full face against light background in civilian clothes taken in a studio (Polaroid photographs and photographs with dark glasses or uniforms are not accepted) should be submitted along with the application. 5.
Minors are issued passports for 5 years or up to attaining 18 years,
whichever is earlier. |

6.
Applicants are advised to ensure that their application in the prescribed
format is complete in all respects; that all columns are correctly and
legibly filled up and no column is left blank or unfilled or vaguely
filled with a dash, tick, etc. All signatures in the application should
be with ballpoint pen in the 2x4,5cm box. |

7.
It is an offence under the Passport Act 1967 to furnish false information
in the application. Passport facilities could be denied on grounds of
suppression of material information, submission of incorrect particulars,
wilful damage/loss of passport or for unauthorised change/tampering.
The Passport can be impounded or revoked for violation of one or more
of the provisions of the Passport Act. |

8.
It is an offence to hold more than one valid passport at a time. |

9.
A passport expired more than six months back should be supported by
a Sworn Affidavit explaining the circumstances and spelling out the
reasons for late submission. |

10.
For change of address, attach documentary evidence such as tax receipt,
electricity/telephone bill or Nativity Certificate issued by the Competent
Authority of the State concerned. |

11.
Indian citizens acquiring citizenship/passport of another country should
immediately surrender their Indian Passport to the Indian Embassy. |

| Issue of New Passport: |
|
Requirements
for issuing new passport are:
|
|
(a) Duly completed application form and Declaration
form filled in BLOCK LETTERS using black/blue ink
only (see Passport
Application Form and Declaration
form);
(b) Two latest identical passport size photographs depicting the
applicant's facial features against a light background;
(c) In cases, where the validity of the earlier passport has expired
beyond a period of one year; the Applicant should submit an affidavit
and may have to appear personally before the Consular Officer;
(d) Copy of Residence Permit or valid visa;
(e) Current Passport;
(f) Proper Fee (see fee schedule for details)

| Passport for a Minor: |
|
Every
child should have a separate passport. The applicant should complete
the formalities as applicable for the issue of a new
passport. The child has to sign or put a thumb
impression (as applicable) on the application form which should
be duly counter-signed by both parents. The validity of such a
passport (for children under 18 years) will be for 5 years. If
the child's name has hitherto been endorsed on the mother's passport,
then the mother also has to fill in a Miscellaneous Service form
requesting deletion of the child's name from her passport. If
the child is born in Norway, the original birth certificate and
parents' passports with a photocopy of each are to be submitted
with the application form.

| Duplicate
Passports (In Lieu of Lost/Damaged Passports): |

Application
must be submitted with the following documents:
|

1. Passport Application form (one copy)
2. Damaged passport or (in case of lost passport) a copy of Police
report;
3. Six identical photographs (facial features against light background);
4. Six copies of Personal Particulars Form;
5. Fee (see fee schedule);
6. In case of student, a letter of verification from school/college
is required;
7. Affidavit;
8. Photocopy of the previous (lost) passport (if available);
9. Photocopy of Driving License or other personal identification document,
preferably with photograph of applicant;
10. Photo copy of Resident Permit or valid visa;
11. Photocopy of documentary proof of residence in Norway.
12. A written declaration, detailing the circumstances in which the
passport was lost/damaged.

| Change
of Name: |
|
Before
change of name/surname, in the passport, the intention to change
has first to be advertised in a prominent daily newspaper, in India
to be circulated in the area of your permanent and present residence.
Original newspaper clippings and a Court Order issued by a judicial
officer not below the rank of First Class Magistrate in India or
a duly notarized affidavit by the applicant are required to be submitted.
(All other requirements are as for the issue of a new
passport booklet.)

| Change
of name / surname after marriage:: |
|
For
change of surname, consequent to marriage / divorce, duly completed
Passport Application Form along with original and a copy of divorce/marriage
certificate and prescribed fee should be submitted. In case the
spouse is a foreign national, a photocopy of the foreign passport
should be submitted along with the application form.

| Correction
in date of birth / place of birth:: |
|
Application
for correction in date of birth/place of birth in the passport is
entertained only when it is accompanied by a Court Order issued
by a judicial officer not below the rank of First Class Magistrate
in India. (Process for the issue of a new passport booklet is to
be followed).

| Extension
of short validity passport: |
|
In
cases, where the passport has been issued valid for one year or
for a limited period a passport can be extended up to the normal
validity of 10 years. For this, Application
Form for Extension of Short Validity Passport, along
with two photographs should be submitted. In such cases, the validity
will be extended only on receipt of clearance from the original
Passport Issuing Authority. No fee is required in this case except
for Czech Korun. 500 as reference charges and the mailing charges.
Applicants should make enquiries only after thirty days from the
date of submission of the application.

| Emergency
travel documentt: |
|
An
Emergency Certificate (one-way travel document) which authorizes
an Indian citizen to enter India, is issued to individuals who have
lost their passports or their passports have been stolen or damaged
and to whom new passports cannot be issued without approval from
India. This document is issued primarily with the objective of ensuring
the applicant's return to India in an emergency. Applicants should
fill in the application form as prescribed for the issue of a new
passport (Passport Application Form).
